What you will see
The folder created on a Windows Server 2012 may have incorrect permissions when the “Scan to Network – Create Directory” value is set to “Yes – Create and Set Permission”.
NOTE: This setting is found on the Lexmark Management Console (LMC) > Solutions tab > PrintReleasev2 > Configuration menu.

Make sure to enable the .NET framework 3.5 in the Windows Server 2012.

An LPM executable, that helps set permissions, appears to have a dependency on .NET Framework 3.5.
Lexmark Document Distributor (LDD) installs the .NET Framework 3.5 upon Client Software installation; however, it still requires activation.
